Suburban requires proof of rabies and dog license for all dogs over four months of age.

For puppies under four months, a health certificate from your veterinarian is required, stating that puppy is in good health, free of worms and parasites and has had all age appropriate vaccinations.

Your space in class will not be reserved until application is complete, including submission of proof of rabies, dog license and payment.

Vaccination records and dog license can be submitted on-line with the application, e-mailed to registrar@suburbandogtraining.com or mailed to Suburban.

Payment may be made on-line after completion of the registration form or check may be mailed to Suburban.
